WA State Medicaid Nutrition Coverage Requirements

Nutrition supplements for Medicaid (DSHS) clients are covered only with prior approval by the DSHS medical review board. Some clients, such as cancer patients and children, may qualify for expedited prior approval, which has a faster turnaround time. One of the primary qualifiers for oral enteral nutrition is loss of weight. Patients must be underweight or malnourished to be approved. DSHS recommends trying blended foods that are calorically enhanced before requesting
a supplement.

If you feel you have a need to receive a nutritional supplement, follow these steps to ensure that your request can be properly processed and coverage can be quickly determined.
1. Call your customer service representative and explain the reason for need. Have your current and past weights available.

2. Your customer service representative will send a pre-approval worksheet form to the physician. The physician will be asked to explain in detail why traditional foods will not fulfill your nutritional requirements.

3. When the worksheet is completed by the doctor, we submit to DSHS for approval. This process can take as long as three weeks.
4.Your customer service representative will call you as soon as approval is received. If your request is denied, we may resubmit with additional information.

Please note that we cannot send a shipment until the request is approved by DSHS. The process typically takes up to a month. We understand that this can be a challenge, but your patience is appreciated.
